Lidocaine Hydrochloride undergoes rapid metabolism by the liver, with both metabolites and unchanged drug being excreted via the kidneys. This biotransformation process involves oxidative N-dealkylation, ring hydroxylation, amide bond cleavage, and conjugation. The primary N-dealkylation pathway produces metabolites monoethylglycinexylidide and glycinexylidide, sharing similar yet less potent pharmacological/toxicological effects compared to Lidocaine Hydrochloride. Approximately 90% of administered Lidocaine Hydrochloride is excreted as various metabolites, with less than 10% excreted unchanged. The principal urinary metabolite is a conjugate of 4-hydroxy-2,6-dimethylaniline.
