Lidocaine hydrochloride is swiftly metabolized by the liver, with its metabolites and the unchanged drug being expelled by the kidneys. This biotransformation process encompasses oxidative N-dealkylation, ring hydroxylation, amide linkage cleavage, and conjugation. The primary biotransformation pathway, N-dealkylation, produces the metabolites monoethylglycinexylidide and glycinexylidide, which exhibit pharmacological/toxicological actions akin to, albeit less potent than, Lidocaine hydrochloride. An estimated 90% of administered Lidocaine hydrochloride is excreted as various metabolites, with less than 10% remaining unchanged. The dominant metabolite in urine is a conjugate of 4-hydroxy-2,6-dimethylaniline.
Lidocaine HCl is a potent amide local anesthetic.
With increased dosage, both efficacy and toxicity rise. At sub-toxic plasma concentrations, Lidocaine HCl offers an anti-convulsive effect.
Blood concentrations exceeding 5μg/ml can induce convulsions.
