Lidocaine hydrochloride undergoes rapid metabolism in the liver, with both metabolites and unchanged drug being excreted through the kidneys. The process of biotransformation includes oxidative N-dealkylation, ring hydroxylation, amide linkage cleavage, and conjugation. N-dealkylation, the primary pathway, produces monoethylglycinexylidide and glycinexylidide. These metabolites exhibit pharmacological and toxicological properties similar to Lidocaine hydrochloride but are less potent. Approximately 90% of administered Lidocaine hydrochloride is excreted as various metabolites, while under 10% remains unchanged. The primary urinary metabolite is a conjugate of 4-hydroxy-2,6-dimethylaniline.
Lidocaine HCl: An Amide Local Anesthetic
As the dose increases, both efficacy and toxicity rise. At sub-toxic plasma concentrations, Lidocaine exhibits anticonvulsive effects.
Blood concentrations exceeding 5μg/ml can induce convulsions.
