Lidocaine hydrochloride undergoes rapid metabolism in the liver, with both metabolites and unchanged drug being excreted through the kidneys. The biotransformation process involves multiple steps: oxidative N-dealkylation, ring hydroxylation, amide linkage cleavage, and conjugation. N-dealkylation, a primary biotransformation pathway, produces monoethylglycinexylidide and glycinexylidide. These metabolites exhibit pharmacological and toxicological actions similar to but less potent than those of Lidocaine hydrochloride. Approximately 90% of administered Lidocaine hydrochloride is excreted as various metabolites, while less than 10% is excreted unchanged. The main metabolite found in urine is a conjugate of 4-hydroxy-2,6-dimethylaniline.

As the dosage increases, both its efficacy and toxicity rise correspondingly. At sub-toxic plasma concentrations, Lidocaine exhibits an anti-convulsive effect.
