Lidocaine hydrochloride undergoes rapid metabolism by the liver, with both metabolites and unchanged drug being excreted via the kidneys. This extensive biotransformation involves oxidative N-dealkylation, ring hydroxylation, cleavage of the amide linkage, and conjugation. N-dealkylation, the predominant pathway, produces metabolites monoethylglycinexylidide and glycinexylidide, which exhibit similar but less potent pharmacological/toxicological properties compared to Lidocaine hydrochloride. Approximately 90% of Lidocaine hydrochloride administered is excreted as various metabolites, while less than 10% is excreted unchanged. The primary urinary metabolite is a conjugate of 4-hydroxy-2,6-dimethylaniline.
Lidocaine HCl: A Potent Amide Local Anesthetic
