Lidocaine hydrochloride is rapidly metabolized by the liver, and both its metabolites and the unchanged drug are excreted via the kidneys. The biotransformation processes include oxidative N-dealkylation, ring hydroxylation, amide linkage cleavage, and conjugation. The primary biotransformation pathway, N-dealkylation, produces metabolites monoethylglycinexylidide and glycinexylidide, which possess pharmacological and toxicological properties similar to, albeit less potent than, Lidocaine hydrochloride. Approximately 90% of Lidocaine hydrochloride is excreted as various metabolites, and less than 10% is excreted unchanged. The primary metabolite found in urine is a conjugate of 4-hydroxy-2,6-dimethylaniline.
Lidocaine HCl functions as an amide-type local anesthetic.
As the dosage increases, both the therapeutic effects and toxicity of Lidocaine HCl intensify. At sub-toxic plasma concentrations, it exhibits anti-convulsant properties.
Convulsions can occur when blood concentrations exceed 5μg/ml.
