Lidocaine hydrochloride is rapidly metabolized by the liver, with both metabolites and unchanged drug being excreted primarily by the kidneys. Its biotransformation processes include oxidative N-dealkylation, ring hydroxylation, amide linkage cleavage, and conjugation. The principal biotransformation pathway, N-dealkylation, produces the metabolites monoethylglycinexylidide and glycinexylidide, which exhibit pharmacological and toxicological actions similar to, but less potent than, Lidocaine hydrochloride. Around 90% of administered Lidocaine hydrochloride is excreted as various metabolites, while less than 10% is excreted unchanged. The primary urinary metabolite is a conjugate of 4-hydroxy-2,6-dimethylaniline.
Lidocaine HCl is a powerful amide local anesthetic.
As the dosage increases, both the efficacy and potential toxicity of Lidocaine HCl increase. It exhibits an anti-convulsive effect at sub-toxic plasma concentrations.
Blood concentrations exceeding 5 μg/ml may induce convulsions.
