Lidocaine hydrochloride undergoes rapid metabolism in the liver, with both metabolites and unchanged drug excreted by the kidneys. This biotransformation involves oxidative N-dealkylation, ring hydroxylation, amide linkage cleavage, and conjugation. The major pathway, N-dealkylation, produces metabolites monoethylglycinexylidide and glycinexylidide, which exhibit similar but less potent pharmacological/toxicological actions compared to Lidocaine hydrochloride. About 90% of administered Lidocaine hydrochloride is excreted as various metabolites, with less than 10% remaining unchanged. The primary urinary metabolite is a conjugate of 4-hydroxy-2,6-dimethylaniline.
Lidocaine HCL is a highly effective amide local anesthetic.
As the dosage increases, both the efficacy and toxicity of Lidocaine HCL increase. At sub-toxic plasma concentrations, it exhibits anti-convulsant effects.
Blood concentrations exceeding 5μg/ml can result in convulsions.
