Lidocaine hydrochloride undergoes rapid metabolism by the liver, with both metabolites and unchanged drug being excreted via the kidneys. The biotransformation process includes oxidative N-dealkylation, ring hydroxylation, amide bond cleavage, and conjugation. N-dealkylation, a key pathway, produces the metabolites monoethylglycinexylidide and glycinexylidide, which exhibit pharmacological/toxicological effects similar to, though less potent than, Lidocaine hydrochloride. Approximately 90% of administered Lidocaine hydrochloride is excreted as various metabolites, with less than 10% excreted unchanged. The primary urinary metabolite is a conjugate of 4-hydroxy-2,6-dimethylaniline.
Lidocaine HCl is a highly effective amide local anesthetic.
With higher doses, the anesthetic's effects or toxicity increase, providing an anti-convulsive effect at sub-toxic plasma concentrations.
Blood concentrations exceeding 5μg/ml may induce convulsions.
