Lidocaine hydrochloride is swiftly metabolized by the liver, with both metabolites and the unchanged drug being excreted via the kidneys. The biotransformation process encompasses oxidative N-dealkylation, ring hydroxylation, amide linkage cleavage, and conjugation. N-dealkylation, the primary pathway, produces the metabolites monoethylglycinexylidide and glycinexylidide, which have pharmacological and toxicological actions akin to, but less potent than, Lidocaine hydrochloride. Approximately 90% of administered Lidocaine hydrochloride is excreted as various metabolites, with less than 10% excreted unchanged. The main metabolite in urine is a conjugate of 4-hydroxy-2,6-dimethylaniline.
