Lidocaine hydrochloride undergoes rapid metabolism by the liver. Both metabolites and the unchanged drug are expelled via the kidneys. The biotransformation process encompasses oxidative N-dealkylation, ring hydroxylation, amide linkage cleavage, and conjugation. The predominant biotransformation route, N-dealkylation, produces monoethylglycinexylidide and glycinexylidide metabolites. These metabolites exhibit pharmacological and toxicological effects similar to but milder than Lidocaine hydrochloride. Approximately 90% of administered Lidocaine hydrochloride is excreted as various metabolites, while less than 10% is excreted unchanged. The primary urinary metabolite is a conjugate of 4-hydroxy-2,6-dimethylaniline.
