Lidocaine Hydrochloride undergoes rapid metabolism primarily in the liver, with both metabolites and unchanged drug excreted through the kidneys. This biotransformation process includes oxidative N-dealkylation, ring hydroxylation, amide linkage cleavage, and conjugation. The primary metabolic pathway, N-dealkylation, produces monoethylglycinexylidide and glycinexylidide. These metabolites exhibit pharmacological and toxicological actions akin to Lidocaine Hydrochloride, albeit with reduced potency. About 90% of administered Lidocaine Hydrochloride is excreted as various metabolites, with less than 10% excreted unchanged. The main urinary metabolite is a conjugate of 4-hydroxy-2,6-dimethylaniline.
Lidocaine HCl is a potent amide local anesthetic.
As the dose increases, both its therapeutic and toxic effects intensify. At sub-toxic plasma concentrations, Lidocaine HCl exhibits anticonvulsive properties.
Blood concentrations exceeding 5μg/ml can induce convulsions.
