Lidocaine hydrochloride is rapidly metabolized by the liver, then excreted by the kidneys, both as metabolites and unchanged drug. This biotransformation includes oxidative N-dealkylation, ring hydroxylation, cleavage of the amide linkage, and conjugation. The primary pathway, N-dealkylation, produces the metabolites monoethylglycinexylidide and glycinexylidide, which have pharmacological/toxicological actions akin to Lidocaine hydrochloride, though less potent. Approximately 90% of administered Lidocaine hydrochloride is excreted as various metabolites, with less than 10% excreted unchanged. The primary urinary metabolite is a conjugate of 4-hydroxy-2,6-dimethylaniline.
Lidocaine HCl is an amide local anesthetic.
As the dose increases, the role or toxicity also increases. At sub-poisoning plasma concentrations, it exhibits an anti-convulsive effect.
Blood concentrations exceeding 5μg/ml may induce convulsions.
