Lidocaine hydrochloride undergoes rapid metabolism in the liver, with both metabolites and unchanged drug being excreted by the kidneys. The biotransformation processes include oxidative N-dealkylation, ring hydroxylation, cleavage of the amide linkage, and conjugation. As a major pathway, N-dealkylation produces metabolites monoethylglycinexylidide and glycinexylidide, which share similar but less potent pharmacological/toxicological actions compared to Lidocaine hydrochloride. Approximately 90% of the administered Lidocaine hydrochloride is excreted as various metabolites, with less than 10% excreted unchanged. The primary urinary metabolite is a conjugate of 4-hydroxy-2,6-dimethylaniline.
Lidocaine HCl: A Potent Amide Local Anesthetic
As the dosage increases, both efficacy and toxicity rise, with anti-convulsive effects observed at sub-toxic plasma concentrations.
Blood concentrations exceeding 5μg/ml can trigger convulsions.
